Eligibility for Hospice
A Caring Help Representative will first contact you and go over all of your questions, concerns, and eligibility to be admitted into hospice.
Admission Visit
If you or your loved one is deemed eligible for hospice care, an admitting registered nurse (RN) will schedule a visit for admission into our care. Upon admission, an initial assessment will be conducted to evaluate the patient's physical, emotional, and spiritual needs.
Plan of Care
The assessment will be reviewed by our Medical Director and healthcare professional team who will create a unique and comprehensive plan of care uniquely tailored to meet each patient's needs. The Plan of Care is carefully crafted to ensure that pain and any other symptoms are managed well, enabling the patient to remain comfortable throughout their hospice experience.

Our team of professionals, including RN's and Skilled Nurses, chaplains, Medical Social Workers, Home Health aides, and more, will be there to ensure that your plan of care is carefully followed, and that you are comfortable and supported throughout the process.
